By using the services of Lotus Diagnostics ("Lotus", "we", "our", "us"), including our website (lotuspathlab.in), WhatsApp booking, phone booking, or visiting our laboratory at Shop 5, Chandresh Apartment, Devidas Rd, Borivali West, Mumbai 400103, you agree to these Terms of Service. If you do not agree, please do not use our services.

Lotus Diagnostics provides the following services: • Pathology and diagnostic laboratory testing (blood tests, urine tests, and other clinical investigations). • Home sample collection services within Borivali West, Borivali East, Kandivali, Dahisar, and nearby areas. • Walk-in sample collection at our laboratory. • Digital and physical report delivery via WhatsApp, email, and in-person collection. All laboratory services are provided under the supervision of Dr. Janhvi Ahuja (MD Pathology), our Chief Pathologist and Lab Director.

Lotus Diagnostics is an ISO 15189 accredited pathology laboratory. Our testing procedures, quality control protocols, and reporting standards comply with ISO 15189 requirements. Accreditation details are available for verification upon request.

• You may book tests via WhatsApp (+91 77770 22655), phone call, or by visiting our laboratory. • Home collection appointments are subject to availability of time slots and service area coverage. • You must provide accurate contact details, address, and medical information for proper service delivery. • Bookings can be rescheduled or cancelled up to 2 hours before the scheduled appointment time at no extra cost. • We reserve the right to refuse service if accurate information is not provided or if the service area is not covered.

• Our trained phlebotomists follow strict infection control and hygiene protocols during sample collection. • Single-use sterile needles, vacutainers, and gloves are used for every patient. • Female phlebotomist availability can be requested during booking. • For certain tests, fasting or specific preparation may be required. Our team will inform you of any requirements during booking. • You are responsible for following pre-test instructions (fasting, medication adjustments) as communicated.

• Reports are delivered digitally via WhatsApp and/or email in PDF format. • Hard copy reports can be collected from the laboratory or delivered on request. • Turnaround time (TAT) varies by test: Most routine tests are ready within 12-24 hours; specialized tests may take 24-72 hours; culture tests may take 3-5 days. • TAT is indicative and may vary based on test complexity, sample quality, or unforeseen circumstances. • Critical and abnormal reports are reviewed by Dr. Janhvi Ahuja before release.

• Payment is due at the time of sample collection unless prior credit arrangements are made. • We accept Cash, UPI (GPay, PhonePe, Paytm), Credit/Debit Cards, and Net Banking. • Prices are subject to change without prior notice. The price applicable at the time of booking will be honored. • Refunds for cancelled tests (where sample was not collected) will be processed within 7 working days. • For tests where sample has already been processed, refunds are not applicable.

• Laboratory reports provided by Lotus Diagnostics are for diagnostic and informational purposes only. • Reports should be interpreted by a qualified medical professional. We do not provide medical diagnosis, treatment advice, or prescriptions. • Using our website or services does not create a doctor-patient relationship. • For medical advice, treatment, or interpretation of results, please consult your treating physician. • In case of critical or abnormal values, our team may proactively contact you to recommend immediate medical consultation.

• We maintain strict quality control standards as per ISO 15189 requirements. • All reports are verified by qualified medical professionals before release. • In rare cases of suspected error, you may request a re-test or review. Please contact us within 48 hours of report delivery. • Our liability is limited to re-testing or refund of the test amount. We are not liable for any medical decisions, treatments, or outcomes based on our reports. • We are not responsible for delays caused by factors beyond our control (natural disasters, strikes, technical failures).

• All personal and medical information is treated as confidential. • Reports are shared only with you and your authorized healthcare provider (with your consent). • We do not share your information with employers, insurance companies, or third parties without your explicit written consent or legal requirement. • Please refer to our Privacy Policy for detailed information on data handling.

• All content on lotuspathlab.in, including text, graphics, logos, and images, is the property of Lotus Diagnostics. • You may not reproduce, distribute, or use our content without prior written permission. • The Lotus Diagnostics name and logo are trademarks of Lotus Diagnostics.

You agree not to: • Provide false or misleading information during booking or registration. • Use our services for any unlawful purpose. • Attempt to access our systems or data without authorization. • Resell or commercially exploit our services without permission. • Harass, abuse, or threaten our staff or other patients.

We reserve the right to modify these Terms of Service at any time. The updated terms will be posted on our website with the revised "Last Updated" date. Your continued use of our services after changes constitutes acceptance of the modified terms.

These Terms of Service are governed by the laws of India. Any disputes arising from these terms or our services shall be subject to the exclusive jurisdiction of the courts in Mumbai, Maharashtra.
